These Terms of Use (“Terms”) govern your access to and use of the Site, including any content, functionality, blog posts, online booking, patient forms, and services we make available through the Site.

2. No Doctor-Patient Relationship
Using the Site, contacting us through the Site (including via contact forms or email), scheduling an appointment, or reading any content on the Site does not by itself create a doctor-patient relationship between you and any chiropractor, massage therapist, or other healthcare provider at Balanced Livermore. A doctor-patient relationship is established only after an in-person evaluation at our clinic and execution of any required intake paperwork.

3. About Our Services
Balanced Livermore is a licensed chiropractic and rehabilitation clinic located in Livermore, California. Our doctors are licensed by the California Board of Chiropractic Examiners. We provide chiropractic care, physiotherapy modalities, rehabilitation exercises, massage therapy, and related services. Treatment plans, fees, insurance acceptance, and scheduling are governed by the policies provided to you at the time of your appointment, not by the Site.
Information about insurance acceptance, pricing, hours, and services on the Site is provided in good faith and is believed to be accurate as of the date of publication. Insurance coverage, rates, hours, and services may change without notice; please call (925) 273-7016 to confirm.

4. Online Booking and Third-Party Services
Online booking is provided through a third-party scheduling platform (Jane App). When you book online, you are also subject to the terms of service and privacy policy of that platform. Balanced Livermore is not responsible for the availability, performance, or content of any third-party site, service, plug-in, or integration linked from or embedded in the Site, including but not limited to Jane App, Google Maps, Instagram, Facebook, payment processors, and analytics providers.

5. Privacy and HIPAA
Your use of the Site is also governed by our Privacy Policy, available at balancedlivermore.com/privacy-policy, which is incorporated by reference into these Terms.
Information you provide through the Site about your health (for example, on an intake form or appointment request) is treated as Protected Health Information (“PHI”) under the federal Health Insurance Portability and Accountability Act of 1996, as amended (“HIPAA”), and is handled in accordance with our Notice of Privacy Practices, available upon request at our clinic and online.
Please do not send sensitive medical information through general contact forms or unencrypted email. If you need to communicate specific health information, please call us or use a secure channel provided by our office.

14. Governing Law and Venue
These Terms and any dispute arising out of or relating to them or your use of the Site are governed by the laws of the State of California, without regard to its conflict-of-laws principles. You agree that any legal action or proceeding will be brought exclusively in the state or federal courts located in Alameda County, California, and you consent to the personal jurisdiction of those courts.

If any provision of these Terms is found to be unlawful, void, or unenforceable, that provision will be deemed severable and will not affect the validity and enforceability of the remaining provisions.

16. Entire Agreement
These Terms, together with our Privacy Policy and any clinic-specific consents, agreements, or notices provided to you at the time of treatment, constitute the entire agreement between you and Balanced Livermore regarding your use of the Site, and supersede all prior or contemporaneous communications and proposals, whether electronic, oral, or written.
